What happens to Unohana in Bleach

Is Unohana's compassion for others just an act?

Unohana was feared by several members of the 11th division, as were some of their fellow captains, such as the former captains of the 5th and 7th divisions.

Retsu Unohana (oh ノ 花 烈, Unohana Retsu), formerly known as Yachiru Unohana (oh ノ 花 八千 流, Unohana Yachiru), was the captain of the fourth division in Gotei 13. She was one of the oldest and most experienced captains and was the best healer in the Soul Society and the first person to be named Kenpachi. [2] Her lieutenant was Isane Kotetsu.

The above quotes suggest that she somehow had a change of heart after joining Gotei 13 and founding the 11th Division, but somehow still showing her side.
